ISO blank template for 'apron' for bottle

I'm not sure what it is called, but I am looking for a transparent template for a bottle(i.e., wine bottle, hand soap, etc.) that slips over the top and hangs on the bottle.  I know I have seen something like it on the forum in the past, but cannot seem to find it.  It's similar to a doorhanger.  Would appreciate any help finding one.  Thanks a bunch.....Debbie
