Here are the ones I use, a cwr file and in case you can't use it a template with measurements and placement guide.
I print mine and wrap around a dollar tree bendy straw and tape and cut to size. Inserts beautifully. Dollar tree straws are thinner than any others I've found. Not sure if thats how others do it but thats what works for  me.

You can get the pens from any store, i shop at Target and they have them, sure any office supply store like Staples would have them as well.

i tried making these today. i got some BIC pens from the Dollar Tree. I picked them up as they had no writing on the barrel. this may sound stupid, but i cannot get the end cap off! Should i just try another brand? any suggestions?

The pens I use are Pentel RSVP pens and thats what the package says. They have a cap that screws off soyou can insert the tube. I've attached a picture of  them before adding the insert.

Thanks for the info and template Becky. As soon as I saw your reunion pictures I thought of making these for my family reunion coming up in August. Will have to get myself some of these pens and start playing
